Acknowledgement

I wish to enroll my child or myself ['the Applicant'] for the Ottawa South United soccer programs. We agree to pay the full fees for the Program and understand that we are not entitled to any refund of fees if our child withdraws or is removed for any reason.

Notice of Warning:

There is a potential risk in training and participating in any sport, and we have tried to create a safe environment.

The coach has established rules for participation, and proper conduct on or about the playing field must be followed.

Release and Indemnity

As a registrant or as the parent/guardian of the registrant, I hereby waive any claim or right, and release and agrees to indemnify and save harmless, OTTAWA SOUTH UNITED SOCCER ASSOCIATION and their directors, organizers, coaches, referees, sponsors, affiliates and other officials, regardless of their involvement as employees, volunteers, or paying participants from any and all liabilities arising from or relating to the participation of the registrant in the activities of the named organization.
I also agree to abide by the published rules of the CSA, OSA, EODSA, my League and my Club.

Acceptance of Enrolment Agreement Terms:

I accept and agree to the provisions of this Agreement.

 NOTE: By signing the Registration form you are agreeing to the foregoing provisions, including the general waiver and acknowledgement, which, among other things, limits the liability of the Club in certain areas.
